Self-Measured Blood Pressure (SMBP) Cost-Savings Calculator

Fiscal Note & Return on Investment: BP-Cuff Coverage for Hypertensive Disorders of Pregnancy (HDP)

EDITABLE Fields outlined in red can be replaced with your state’s own data; otherwise the defaults (drawn from published literature) apply.
The only required input is the total number of deliveries in your state. The neonatal figures are needed only if you enable the optional neonatal section.
Two assumption fields (avoidable share and cuff coverage share) default to the values of the base model; lower the avoidable share to test more conservative scenarios.
If your hospital dataset reports charges rather than costs, enter the HCUP cost-to-charge ratio (CCR) in the CCR field. All hospital cost fields are converted to costs (Cost = Charge × CCR).

WIH example: 8,500 deliveries/year and 20% HDP, the population figures from the supplied fact sheet (one hospital’s annual volume, labeled illustrative). Costs stay at the literature defaults; this example does not reproduce the fact sheet’s readmission-based $533,534 figure, which uses a different (readmission) model. · Copy summary copies a plain-text results block for pasting into documents; Print opens the browser print dialog (state name and date are added to the printed header).

INPUT Total number of deliveries in the state
Required. Source: state hospital discharge (HCUP SID), CDC WONDER, NCHS, CMS Medicaid, or birth certificates.
EDIT % of pregnancies complicated by HDP
Default 15% (nationwide average). Enter as a percent, e.g. 15 = 15%.
EDIT Average cost of delivery c/b HDP
Default $25,437 (Hersh et al., 2022; range 16,893–37,261).
EDIT Average cost of uncomplicated delivery
Default $18,865 (Peterson–KFF Health System Tracker).
EDIT Average cost of a blood-pressure cuff
Default $50 (literature range $40–60).
EDIT Share of the HDP cost difference expected to be avoidable (%)
Default 100% = the full HDP cost difference is avoided (the base model), an upper bound. Lower values test more conservative assumptions. This is a scenario assumption, not an observed treatment effect.
EDIT BP-cuff coverage share of all deliveries (%)
Program cost is charged on the covered population (deliveries × this share). Default = HDP rate, matching the base model; set 100 for universal coverage.
EDIT Cost-to-charge ratio (CCR, optional)
Leave empty if your cost inputs are already costs (the defaults are costs). If your hospital dataset reports charges, enter your HCUP cost-to-charge ratio as a percentage (for example, 0.55 becomes 55%); every hospital cost field is converted (Cost = Charge × CCR). The cuff price is a device cost and is not converted.
Number of HDP deliveries
—
= total deliveries × HDP %
Total cost of deliveries c/b HDP
—
= HDP deliveries × cost c/b HDP
Total cost if uncomplicated
—
= HDP deliveries × cost uncomplicated (counterfactual)
Difference in hospitalization / readmission cost
—
= total c/b HDP − if uncomplicated
HDP cost difference expected to be avoidable
—
= cost difference × avoidable share %
BP-cuff program cost
—
= deliveries × coverage share × cuff cost
Maternal cost savings
—
= avoidable cost difference − BP-cuff program cost
INPUT Number of neonates affected by HDP
Source: same data sources as deliveries above.
EDIT Average neonatal cost, c/b HDP
Default $40,000 (2025 estimate ~$35,000–45,000; Hao 2019; Li 2021; Stevens 2017).
EDIT Average neonatal cost, uncomplicated
Default $3,700 (Hao et al., 2019; Li et al., 2021).
Neonatal cost c/b HDP
—
= neonates × neonatal cost c/b HDP
Neonatal cost if uncomplicated
—
= neonates × neonatal cost uncomplicated (counterfactual)
Total additional neonatal cost savings
—
= (neonatal c/b HDP − if uncomplicated) × avoidable share %
Total cost savings (maternal)
—
Estimates for advocacy illustration only. Each state must supply its own values from its own data. Figures are literature-based defaults (see sources) and are not a quote, guarantee, or projection of actual savings. The headline savings assume the full HDP cost difference is avoidable unless the “avoidable share” field is set lower; treat the headline as an upper bound until your state justifies a specific assumption. Where hospital datasets carry charges rather than costs, enter your HCUP cost-to-charge ratio (CCR) in the CCR field. The calculator converts all hospital cost fields (Cost = Charge × CCR).

Data sources (for state-specific values)

American College of Cardiology
State hospital discharge data (HCUP State Inpatient Databases / SID); CDC WONDER; National Center for Health Statistics (NCHS); CMS Medicaid data (requires approvals); state hospital discharge datasets; birth certificate data (CDC National Vital Statistics System). Identify HDP via ICD-10-CM and obstetric DRGs; flag readmissions; convert charges to cost via CCR.
